New Patient Forms and Consultation

During your first visit, we get to know you and you get to know us!

Upon your arrival to the office, you will be extended a warm welcome!  We will invite you to share specific health and wellness information and goals by completing our patient forms.  A copy of your insurance card and identification will be taken at this time so please bring these items with you.  Your chiropractor will then review your current and past health history with you for accuracy and any additional needed details in our consultation.  It is at this time that you may also discuss your health related problems, concerns, and potential healthcare options.

Physical Examination and Radiographs

After your consultation, we will perform a complete physical and chiropractic examination to include a dynamic neuro-spinal assessment. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing  will be done in our office without your consent.  Following your examination, depending on your specific presentation, you may be scheduled for radiographs (xrays) to be taken.  X-rays help us to develop the most effective treatment plan for you and alerts us of any serious spinal conditions.  The doctor carefully weighs the benefits and risks of these tests and orders them if necessary.  In my experience, I've found that many patients are amazed once they see their very own xray films and in some cases can identify the areas of misalignment and degeneration themselves.  At the conclusion of the examination, if no xrays are warranted, we will move forward with the Report of Findings as stated below to discuss your examination results and doctor recommendations.  Once all your questions are answered, you may receive your first adjustment on the very same day of your exam immediately jump starting your Road to Wellness program!

Report of Findings and Collaboration

When you return, the doctor will explain all exam findings and discuss any recommendations for care. Most people when going to the doctor have three questions:

  1. How can you help me?
  2. How often do I need to come in and for how long?
  3. What will  my care cost?

After reviewing your health history, goals, and examining your spine, we will further discuss specific recommendations, your questions, and notify you if  you  require care with other providers.  If it is determined that chiropractic care is an option for you then recommendations for chiropractic treatment are suggested as well as recommendations to include other healthcare disciplines into your care plan to co-manage and work alongside for the success of your health.
If PEC will be filing insurance for you, your coverage will be reviewed and payment options explained at this time.

Ultimately, no matter who we are, all of us will eventually deal with some degree of fatigue, pain, stress, ailment or injury.  While traditional medicine has it's place and offers great options for many conditions, the downside is that it often offers medication to ease the symptoms associated with various illnesses or injuries.  By masking the symptoms, patients often don't end up addressing the root or cause of the problem until it balloons into something else or worse.  While at other times, some ailments simply don't respond to medication like chronic pain, headaches, etc. When you find yourself in this position and desiring something new because the "same old thing" just isn't enough, then visit us here at PEC.  Allow us to work with you to improve your quality of life, your health and longevity, and your physical and mental outlook.
